SUMMARY OF THE NOVEL CONTRIBUTIONS OF THE DOCTORAL DISSERTATION

Novel contributions of the dissertation:

- The dissertation provides updated local data on antimicrobial resistance in 350 H. pylori isolates collected at 108 Military Central Hospital during 2019-2022. Resistance rates to clarithromycin, metronidazole, levofloxacin, amoxicillin, and tetracycline were 44.6%, 44.0%, 33.4%, 30.6%, and 9.1%, respectively. Overall, 82.0% of isolates were resistant to at least one antibiotic, and 20.3% were resistant to three or more of the five antibiotics tested. These findings provide clinically relevant evidence for local resistance surveillance and treatment selection.

- The study further defines genotype-phenotype associations underlying antimicrobial resistance. The 23S rRNA variants A2143G, G2212A, A2223G, C2288T, and A2302G were independently associated with clarithromycin resistance. N87K/I, D91G/Y/N, and R140K in gyrA were independently associated with levofloxacin resistance. F366L and S414R in pbp1 were independently associated with higher odds of amoxicillin resistance, whereas D508N/E/K was associated with lower odds. In contrast, metronidazole resistance appeared complex and multigenic and could not be adequately explained by a single frxA variant.

- The study provides the first concurrent assessment in Vietnam of the adhesion genes babA, homA, and homB in relation to gastric diseases and antimicrobial resistance. babA was detected in 73.7% of isolates and was associated with gastric ulcer. Multivariable analysis showed that homA and homB were independently associated with amoxicillin resistance.

- Whole-genome sequencing (WGS) was performed on nine purposively selected isolates representing different resistance phenotypes, enabling integrated characterization of resistance-associated variants, virulence factors, and molecular epidemiological features. Three novel multilocus sequence types - ST4511, ST4512, and ST4513 - were identified and registered in international databases. These findings expand genomic and molecular epidemiological data on H. pylori in Vietnam and provide a foundation for larger-scale genomic studies.
