Tóm tắt những đóng góp mới của luận án:

Đề tài có nhũng đóng góp mới có giá trị như sau:

*Về đặc điểm lâm sàng, cận lâm sàng và các chỉ số siêu âm đánh dấu mô cơ tim ở bệnh nhân bệnh thận mạn tính có phân suốt tống máu thất trái bình thường.

- Bệnh thận mạn tính có mức lọc cầu thận trung bình 21,02 ml/phút/1,73m2, Đái tháo đường: 23,7 %, tăng huyết áp: 55,3 %, rối loạn lipid máu: 67,4 %, thiếu máu: 87,4 %, tăng NT-proBNP: 61,6 %, PĐTT: 65,8 %, Rối loạn chức năng tâm trương: 62,8 %, tăng áp lực động mạch phổi: 43,7 %.

- Siêu âm đánh dấu mô cơ tim: GLS giảm 37,9 %, MSP giảm: 16,3 %, GLSRe giảm: 13,7 %, MDP giảm: 11,1 %. Các chỉ số biến dạng xoay xoắn tăng hơn nhóm chứng chưa có sự khác biệt

*Về mối liên quan giữa các chỉ số siêu âm đánh dấu mô cơ tim với đặc điểm lâm sàng, cận lâm sàng ở bệnh nhân bệnh thận mạn tính có phân số tống máu bình thường.

- Liên quan với giai đoạn bệnh thận: Các thông số biến dạng tâm thu, tâm trương giảm dần theo mức độ bệnh, có tương quan thuận với mức lọc cầu thận (p <0,01), điểm cắt mức lọc cầu thận: 26,24 ml/phút/1,73m2 có giá trị dự báo suy chức năng thất trái. Bệnh thận mạn tính có tăng huyết áp, đái tháo đường có các chỉ số biến dạng giảm, tỷ lệ suy chức năng tâm thu, tâm trương tăng hơn ở nhóm bệnh thận mạn tính không có đái tháo đường, tăng huyết áp. Bệnh thận mạn tính có phì đại thất trái: Các chỉ số biến dạng cơ tim giảm, tỷ lệ rối loạn chức năng tâm thu, tâm trương tăng.

- Phận tích đa biến: Tăng huyết áp là yếu tố tiên lượng độc lập dự báo giảm chức năng tâm thu, tâm trương ở các giai đoạn bệnh thận mạn tính.

- Kết quả nghiên cứu cho thấy ở bệnh nhân bệnh thận mạn tính có phân số tống máu thất trái bảo tồn có giảm cả chức năng tâm thu và tâm trương. Tuy tỷ lệ giảm chưa cao, nhưng lại là biến đổi sớm. Đây chính là đóng góp có ý nghĩa của đề tài cho chuyên ngành.

Summary of new main scinetific contribution of the thesis:

The topics with new contributions are as follows:

* In terms of clinical, subclinical and myocardial deformation in patients with chronic kidney disease with a normal left ventricular ejection fraction.

- Chronic kidney disease has an GFR: 21.02 ml/min/1.73m2, Diabetes: 23.7%, hypertension: 55.3%, dyslipidemia: 67.4%, anemia: 87.4%, increase NT-proBNP: 61.6%, Left ventricular hypertrophy: 65.8%, Diastolic dysfunction: 62.8%, Hypertension pulmonary artery: 43.7%.

- Myocardial deformation: GLS decreased 37.9%, MSP decreased: 16.3%, GLSRe decreased: 13.7%, MDP decreased: 11.1%. The rotation and twist of left ventricular increased more than the control group without any difference.

* The relationship between the myocardial deformation with clinical and subclinical features in chronic kidney patients with a normal ejection fractions.

- Related to the stage of kidney disease: The systolic and diastolic parameters decrease with the grade of disease, positively correlated with the glomerular filtration rate (p<0.01), the glomerular filtration rate cut-off: 26.24 ml / min / 1.73m2 has predictive value of left ventricular dysfunction. Chronic kidney disease with increased blood pressure, diabetes with decreased deformation index, the rate of impaired systolic function, diastolic increased more in the group of chronic kidney disease without diabetes, increased blood pressure. Chronic kidney disease with left ventricular hypertrophy: Myocardial deformation index decreases, the rate of systolic and diastolic dysfunction increases.

- Multivariable analysise: Hypertension is an independent predictor of decreased systolic and diastolic function in the stages of chronic kidney disease.

- Research results show that in chronic kidney patients with a normal left ventricular ejection fraction, there is a decrease in both systolic and diastolic function. Although the rate of reduction is not high, it is an early change. This is the meaningful contribution of the topic to the major.
